The AFC Fylde Community Foundation are proud to be strengthening its links with Fylde and Wyre Clinical Commissioning Group, thanks to support provided by The Football Conference Trust. Over the past twelve months, The AFC Fylde Community Foundation have been delivering a small pilot project called, ‘Get on Board’. This project was designed to deliver an early intervention education project, which residents of all ages were provided with the opportunity to take part in, and improve their health and wellbeing. This twelve month project, involved a total of 106,154 Fylde residents, engaging them in activities such as; Primary School Education Programmes, Community Family Engagement and education programmes, Workplace fitness programmes and 50+ Social Isolation combatting programmes. These programmes produced fantastic results, which truly benefitted the Fylde Coast.

Given the success demonstrated through partnership working between Fylde and Wyre Clinical Commissioning Group and The AFC Fylde Community Foundation, delivering the ‘Get on Board’ programme, a stronger, more diverse partnership has been formed. The AFC Fylde Community Foundation, working in partnership with The Football Conference Trust, have successfully brought in a substantial amount of funding to deliver the newly reformed and developed ‘Get on Board’ health intervention programme. Given the success of the initial Health Improvement project, delivered for a total of twelve months, The AFC Fylde Community Foundation will be delivering the original streams originally within the ‘Get on Board’ programme, plus new and exciting additional streams which will deliver intense intervention streams, working with some of The Fylde Coasts most vulnerable residents.

The ‘Get on Board’ health improvement programme will be commencing delivery in September 2015.
