Role Summary: 

The AFC Fylde Community Foundation is a charitable community trust, based in Kirkham Lancashire. The Foundation, working closely with AFC Fylde Football Club, deliver a range of Community Enhancement activities to a wide and diverse range of participants, throughout Blackpool Wyre and Fylde. The Foundation, overseen by its HWC delivers a range of Public Health improvement programmes, namely the ‘Get on Board Programme’ which the HWO will be working within.

Guidance and Authority: 

The HWO will work closely with the Foundations HWC and DCD, who will assist and develop a range of delivery projects, throughout Blackpool Wyre and Fylde. The HWO will report directly into the Foundations HWC on a weekly basis.

Main Duties: 

Identify, contact and develop relationships with schools and individuals from agreed target groups and communities

  1. Develop work through listening and exploring ways to respond in relation to locally identified need and potential
  2. To identify the needs of schools, participants and groups to put together a work plan including objectives, outputs and outcomes relevant to appropriate timescales.
  3. Liaise with local sports and activities clubs and leisure providers to provide a range of exit strategies for individuals from the programme

 

Programme Development and Delivery:

  1. Deliver AFC Fylde “Get on Board” programme and Family engagement programme
  2. Deliver AFC Fylde alcohol awareness programme
  3. To produce up to date session plans for all delivery
  4. To complete all Risk Assessment procedures and protocols

 

Partnership working:

  1. To work through a partnership approach to identify the barriers preventing easy access to services for local people within their locality;
  2. Work in partnership with other workers and agencies as necessary;

 

Freedom to act:

  1. Independently organise work plan, information storage, retrieval and collation systems, diary and working time (including out of hours)
  2. Regular lone working in the community

 

Communication:

  1. Use of highly developed culturally competent communication skills with community members and organisations to negotiate, influence, and resolve conflict that may arise

Facilitate the involvement of community members in the reporting of needs, outputs and outcomes

 

Occasional Duties: 

The post-holder may be required to:

  • Work additional or anti-social hours as required to meet the needs of the business
  • Attend staff meetings and training as needed
  • Travel to other sites
  • Carry out any other duties as are within the scope and purpose of the job as requested by the CSM

 

Personal Attributes:

The post-holder will have:

  • A sound knowledge of nutrition and physical activity themes.
  • Flexible approach to work
  • Experience working with young people in an educational environment, which includes both high and primary schools.
  • Experience in partnership working, delivering public health programmes within nationally recognised guidelines
  • Experience and/or knowledge of the Community Sector, which includes delivering, funded projects in a timely and accurate manner.
